With those results, I’d stick with a nitrogen only fertilizer. Additionally, iron will help get a darker green.

Are you able to spray with a backpack sprayer or pump sprayer? If so I’d go with AMS or urea. AMS will help get the ph a little lower over time.

How does the grass look now?

I do have a backpack sprayer and I just got my order of FEATURE 6-0-0 today

It looks like it’s doing pretty good as far as I can tell. It’s very possible you’ll only need to apply nitrogen and your feature this season as far as fertilizers go.
 
It looks like your soil test might have fertilizer in it. You should wait at least 4 weeks after fertilizing to get accurate soil tests. Your pH will be fairly accurate though.

You're beyond high for everything, including sulfur. Sulfur needs to be under 100ppm.

Hold on fert for a few weeks. Then continue N only, like urea. I'd be afraid to say ammonium sulfate with that much S.

You can start reducing pH by top dressing with peat moss and spraying citric acid frequently.
